Patients can now receive DIRECT ACCESS to physical therapy services without a referral from a physician. The Michigan Legislator has passed a new law allowing patients to seek physical therapy treatment without a physician referral (Senate Bill 0690 – effective January 1, 2015).

Plymouth Physical Therapy Specialist is committed to the highest level of patient care, including the most up-to-date treatment approaches, state of the art equipment, and same day appointments.

On your first visit you will be greeted by a member of our front office staff who will orient you to our practice. New patient paperwork can be printed (below) and completed prior to coming in for your appointment. If you prefer, you may arrive 15 minutes prior to your appointment to fill out the forms in the clinic. Please wear comfortable, loose fitting clothing for your evaluation. You will also need to bring in your driver’s license, insurance cards, and the prescription provided to you by your physician.

The physical therapist will complete a thorough evaluation and discuss a plan for treatment with you. The therapist will recommend how often you should attend physical therapy and our front office personnel will assist you in scheduling all of your appointments at that time. Please remember to check out at the front desk after each visit.
